NOTE: Please be advised that Field Placements are available to Ontario residents only.

Field placement instructions

Please complete the following steps once you have registered for your field placement or clinical course:

From the list of quick links below:

  • Select, print and thoroughly review the Information Sheet that pertains to your Program of Study. (Follow ALL directions indicated and submit ALL required documentation indicated in this document by the submission deadlines above)
  • Select, print and complete the Field Placement Application that pertains to your Program of Study
  • Select, print and complete the Entry Immunization Form (required for all Field placements and clinicals)

Please note: It is a now a requirement of all Field Placement students to purchase a photo I.D. badge. The I.D. badge must be worn while attending your assigned hours at your placement site. Please refer to the Field Placement Information document for your Program for directions on how and where to obtain your photo I.D. badge.
