Change of Information

If you change your address or telephone number while you are employed at Durham College, please complete the Change of Information Request Form and submit it to Human Resources for processing.

Should you experience a personal or employment status change that may affect your benefit coverage, such as change of name, change in marital status, birth, death or beneficiary designation, please contact Human Resources within 31 days. In order to ensure appropriate benefit coverage it is important that your employee records are accurate and up-to-date.
