How to make an appointment

For all health concerns please book an appointment by calling 905-721-3037 or stop by the Campus Health Centre during operating hours.

During your FIRST visit you will be required to complete an:

• Initial Health History Form - PDF 85KB and;
• Guidelines for Patient Use of Medical Services - PDF 83KB.

Please complete these forms and bring them with you to your appointment. Remember, you need to complete this form only ONCE on your first visit.
