Procedure for booking an appointment with a physician

• To book an appointment at the Campus Health Centre we ask that you call 905-721-3037 or e-mail health.wellness@dc-uoit.ca.
• For the appointment you must bring your student ID card as well as an insurance form.
• You will see the physician first.
• After seeing the physician you will be asked to stop at the reception desk.
• At that time the clerk will ask that you make payment. We accept cash, debit, Visa, Mastercard, or American Express.
• We are unable to inform you of the exact cost of the appointment prior to seeing the physician; The physician must see you first.
• You will be billed for the physician visit as well as anything else required at that time e.g. blood work, injection etc.
• Upon payment you will be given a receipt.
• The receipt and completed form must be mailed, by you, to the insurance company.
• Address for the insurance company is on the form.
• You are advised to make a copy of everything (the form and the receipt) for your own records.
